The Paramount Case Management team is currently recruiting for a Clinical Outreach Coordinator. The main function of the Clinical Outreach Coordinator is to provide support to case managers with medical cases (Accountable Point of Contact). They also work within their scope of practice as an integral part of the interdisciplinary team under the direction of the case manager to interact with members providing education, community resource planning, and support. Duties include, but are not limited to:

Contribute to the assessment process by gathering information from members, family members, and providers. 

Report visit information back to the case manager to assist with the development, implementation, and modification of the care plan.

Participate as a team member to collaborate in the care planning process.

Provide home and community visits to high risk members and their families in an effort to provide education, support, and guidance to improve outcomes in a cost effective manner.

Assist members on understanding plan benefits, accessing needed services, importance of participating and adhering to the treatment plan.

Contribute to coordination of care, facilitation of services, and providing education to members.

Perform documentation in the electronic medical record.

Perform other duties as directed.

Job Requirements

Current Ohio Licensed Practical Nurse or Social Worker license required.  

Valid driver’s license with proof of auto insurance. 

Must have the ability to travel and make face-to-face visits with members. Visits may take place in a variety of settings; including but not limited to member home/provider office.  

Managed Care and Home Care experience preferred.

Excellent communication (oral and written) and organizational skills required. 

Demonstrated ability to plan, coordinate and organize multiple priorities.

Demonstrated PC skills, including word processing/spreadsheet and applications strongly preferred.  Experience with electronic health record documentation.

Ability to move between company work stations and departments; ability to communicate on telephones and operate general office equipment, including computers.
